STUDENT APPLICATION PROCESS

STEP 1: FILL OUT THE BELOW APPLICATION

Student Application – Each student wishing to be considered for admission to UTAH PREP must fill out and complete the online application below and submit the required documentation explained further in STEP 2.

STEP 2: PROVIDE THE REQUIRED DOCUMENTATION

Required Documentation – Requested documentation may be uploaded at the link provided at the end of the application. Each student must provide the following items as applicable:
1) Age Verification & Immunization Records – Please provide a certified copy of your of birth certificate & immunization records. Immunization records must be from your physician or applicable national or state authority, and dated within one year. Birth certificates and immunization records do not have to be translated into English.

2) Academic Records – Please provide an official school transcript of your previous years in high school (Grades 9-11). Incoming Freshman (grade 9) will be required to provide their previosu year’s transcript. Transcripts must include the school’s seal.

3) Copy of Passport – If applicable, please provide any assessments or evaluations such as an IEP, 504, or Psycho-Educational Evaluation.

If applicable (for international students), please provide your most recent TOEFL, IELTS, or Duolingo results.

FOR INTERNATIONAL STUDENTS

4) Visa Application – All non-US citizens wishing to study in the United States need a Form I-20, “Certificate of Eligibility for Nonimmigrant Student Status.” Once accepted into our Student and Exchange Visitor Program (SEVP) you will receive a Form I-20. Assistance will be provided upon request in filling out the Form I-20 visa application.

5) Bank Statement or Official Bank Letter Verifying Sufficient Funds – Please provide the necessary banking documentation that wil be required for granting of your visa. You may contact your admissions representative for assistance with this. (View sample in required documents checklist link below)

STEP 3: ACCEPTANCE & ENROLLMENT

Once your application is completed, you will receive a decision notification from Utah Prep’s Admissions Department. Upon approval you will receive an Electronic Enrollment Agreement to be completed and returned with the required deposit. All payments can be paid via credit card, check, or wire. Please indicate the payment option in the Utah Prep Student Application and your choice of payment plan (Pay in Full or Payment Plan). Merit based financial aid and scholarships are available on a case by case situation.
